Marginal periodontitis with grade three furcal lesion and a pocket extending to the apex in the distal root of tooth 36 were diagnosed in a 67-year-old man. Sensitivity test revealed the tooth to be necrotic. The tooth showed only slight mobility. A decision was made on hemisection and removal of the distal root after endodontic treatment of the mesial root canals.
