Acute pulpitis was diagnosed in tooth 47. The tooth had relatively deep fillings but no sign of primary or secondary caries. Preoperative radiograph showed two roots and at least two root canals. The cause for pulpitis was probably earlier caries and restorative procedures. Mandibular block anesthesia was applied and the pulp chamber was perforated with a high speed diamond bur under water cooling. The rest of the pulp chamber roof was removed with a round bur at low speed. Preliminary examination of the chamber floor with a probe showed one distal and two mesial root canals.
