Acute pulpitis was diagnosed in tooth 42. Strong attrition was the probable cause for the pulpal inflammation. Preoperative radiograph taken from the lower incisors showed attrition in all front teeth, previous root fillings of questionable or poor quality were also detected in central incisors. Two root canals were clearly seen in tooth 42. Local infiltration anesthesia was applied and pulp chamber was perforated with a high speed diamond with water cooling.
