In order to evaluate the long term prognosis of the tooth, gutta-percha points were introduced into the pocket to measure the extend and depth of the periodontal lesion around the tooth, and a radiograph was taken. Apicoectomy was not possible because of loss of marginal bone. The diagnosis was symptomatic apical periodontitis with a fractured instrument, and the treatment plan was a conservative treatment of a non-vital tooth
