Four weeks later the sinus tract still persisted and there was pus coming out from the distal pocket. The canal was prepared to size 90, amoxicillin therapy was started and new calcium hydroxide was packed in the canal, sealed with IRM. Bacteriological sample was taken and a antibiotic treatment with amoxicillin & metronidazole was started. Three weeks after the latest antibiotic treatment, the sinus tract was still open and pus was coming from the pocket. Surgery was then done and apical and lateral granulation tissue was removed. The retrograde cavity was filled with IRM together with the lateral resorption cavity.
