A 40-year-old male patient was referred to an endodontic specialist because of a fracture Lentulo instrument in the distal canal. The original diagnosis of the tooth was pulpal necrosis. A radiograph revealed a fractured instrument in the apical half of the distal canal and some radio-opaque material coronally, probably calcium hydroxide. The patient was symptomfree.
