A forty-eight-year-old man was referred to endodontic retreatment of tooth 16. The patient had complained mild pain in the area. Previous endodontic treatment had been completed ca. six years earlier. Clinical examination showed tooth 16 with a MOD composite filling. The tooth was symptomatic to palpation and percussion. Periodontal probing depth was within normal limits. Radiographically, three root fillings were seen together with an apical lesion around the root apex of the mesiobuccal root. The diagnosis was symptomatic apical periodontitis and the treatment plan was retreament of tooth 16.
