Apical periodontitis with a buccal sinus tract was diagnosed in a 40-year-old man. The radiograph showed three root fillings that were short and probably leaking. Diffuse lesions could be seen at the periapical area of both distal and mesial root. The pulp chamber was only partially filled with gutta-percha and sealer.
