Tooth 12 had been treated and rootfilled in two appointments that were two weeks apart. At the third appointment endodontic surgery had been done to remove the inflammatory apical tissue and to cut the root apex. The lesion had perforated both the buccal and palatal cortical bone.

PAD from the periapical sample revealed a radicular cyst as the final diagnosis. All symptoms disappeared after the operation.
