A forty-one-year-old woman was referred to an endodontic specialist because of root perforation during post space preparation in tooth 45. The perforation site had been filled with calcium hydroxide and sealed with IRM filling coronally.

At the time the patient attended the specialist the tooth was sensitive to percussion.
