Chronic apical periodontitis was diagnosed in tooth 32 in a 40-year-old patient with diabetes. A wide pocket / sinus tract that could be probed to the apex was observed in tooth 41 together with some periapical and mostly lateral bone destruction. Sensibility test revealed that both teeth were necrotic. That is why it was assumed that the deep "pocket" in tooth 41 was of endodontic origin and no periodontal treatment should be done in order not to destroy the periodontal ligament.
