A forty-nine-year-old man came acute for treatment of tooth 25. Diagnosis was acute apical peridontitis, and emergency treatment was eugenol pellet with IRM topfilling.

One week later the patient returns with pain and swelling both buccally and palatally. Buccal incision was performed under local anesthesia, pus was draining out the incision.
