A thirty-five-year-old woman was referred to endodontic retreatment of tooth 37. The tooth was asymptomatic to palpation and percussion at the time of the first appointment, but had earlier presented symptoms. Periodontal probing depth was within normal limits. Radiographically, silver points were seen in two meaisl canals while a screw intraradicular post and a gutta-percha root filling were seen in the distal canal. A periapical lesion of 7 mm surrounded the mesial root apex of the tooth. The diagnosis was asymptomatic apical periodontitis and the treatment plan was retreatment of tooth 37.
