A 34-year-old Caucasian male attended dental care because of pain in the upper left first molar. A clinical investigation revealed a deep caries lesion, probably extending to the pulp. Infiltration anaesthesia was applied and the caries was removed with high-speed diamond bur and large round bur at slow speed. Caries perforation to the pulp was verified and acute pulpitis was recorded as the diagnosis. An eugenol pellet was placed in the pulp chamber and the access cavity was sealed with zinc oxide eugenol filling. Pulpectomy was not started at this stage because neither the patient nor the dentist had scheduled time for this.
