The symptoms had disappeared immediately after the first visit. Two weeks later the patient returned for pulpectomy of tooth 26. The symptoms were returning gradually but were only mild. Infiltration anaesthesia was applied, the rubber dam was placed and the area was disinfected with chlorhexidine-ethanol (0.5 % / 70 %) solution. Four root canal openings were found (pal, d, mb and mp). In addition, a deep groove of 5 - 7 mm was detected between palatal and distobuccal canals.
