Another projection of the mesial canals showing that they had their own apical foramen. Notice that the distal groove was not so clear in this projection. Apex locator and working length radiographs were used to determine the correct instrumentation length for the four canals. The canals were then instrumented as follows: PAL 60 / 21 mm; DB 45 / 20 mm; MB 40 / 20.5 mm; MP 40 / 20 mm. Control of bleeding and removal of residual pulp tissue was technically difficult and time demanding in particular from the groove connecting the pal and db canals. Calcium hydroxide was placed in the canals with a Lentulo at low speed and the access cavity was sealed with Cavit-G and IRM.
