Healing with scar tissue

Sometimes in previously root filled teeth irregularly shaped lesions may be seen, which are not caused by apical periodontitis or other inflammatory processes. These lesions are scar tissue, i.e. healing of bone has not occurred by normal bone formation but, instead, by formation of a sterile, inflammation-free scar tissue. This type of healing is most frequently found in apicectomised teeth where the lesion (and surgery) have perforated both the buccal and palatal/lingual cortical plates of the bone. Depending on the radiographic projection, scar tissue can often be seen at some distance from the root tip. If the root tip is surrounded by PDL and a bone bridge, diagnosis is straightforward.

These teeth are symptom-free and no treatment is indicated.
